All patients please fill out the intake form after booking your eye exam and or medical appointment. This will be sent to you by email.
If you are wanting the dilation drops after your eye exam, please call us to book the drops specifically.
Our clinic will remain by appointment for all visits. We also ask that patients do not arrive more than 10 minutes early for their appointments, unless you are getting a dilation only that requires you to arrive 30 minutes early.
If you are waiting for Covid test results or have any flu/cold symptoms, please call us to reschedule your appointment.
- If you will be 10-15 minutes late or earlier than your appointment time, please call our clinic so we can make sure everyone is seen on time.
- We ask that all patients check their insurance coverage before their appointment, to make sure they have active coverage for the eye exam, glasses/contact lenses. We can only predetermine coverage amounts through Blue Cross.
-Please do not wear contacts into your appointment. You can bring them and wear glasses if possible.

For persons 0-18 years of age (Exam Covered by Alberta Health Care) Please wear glasses not contacts into appointment
This is for new medical eye health issues only. Please call to book dilation drops & follow up app's. (i.e; red irritated eyes, object in eye) Call if you are having flashes of light in vision
If we have called you to let you know your glasses and or contact lenses are ready for pickup. If you also need to pick up anti-fog cloth, cleaning products or dry eye products.

If you are needing your frame tightened, new nose pads or frame has broke and needs a repair. Also if we have called you to let you know your replacement frame/part is here.
If we have called you to let you know your new lenses have arrived and need to be cut into your own frame. Please bring your own frame so we can check your vision after.
For persons 0-18 years of age (Exam covered by Alberta Health Care) If child wears contacts there will be a fee of $40 to have them checked & updated. Please wear glasses into appointment
